You can gain weight in Once Human by eating food items with Corn Oil or such fatty ingredients. Alternatively, you can also carry high weight for a longer time in your inventory to increase your body weight.

In Once Human, you can lose weight by overeating food until you start puking or engaging in activities that require stamina, gradually burning that extra fat and lowering your body weight. You can also do push-ups to gain muscle and lose weight.
